Most of the pictures on website are our patients, their parents, and our staff. We sincerely thank them for giving us permission and the rights to publish them. Any copying (in any form) of these pictures, is a violation of HIPPA. We have purchased the rights to the other stock pictures we have used.

What Information Do We Collect?

Automatic Collection of Information:
If you do nothing during your visit but browse through the web site or download information, our system will automatically gather and store certain information about your visit. • At different places on our site (such as Secure Online Appointment Request Forms and Secure Online Prescription Refill Request Form) you will be asked to provide personal and health-related information such as your name, telephone number, e-mail address, your child's name, date of birth, your child's health information and health history. This information is used to process your online request.

• Personal information/identification information gathered through registration for educational programs
